Looking for new client paperwork or other forms for Champaign Counseling?

We use a HIPAA compliant forms manager that will allow you to sign in to our portal.  This allows us to collect your information electronically and keep your private information secure. Here's our Notice of Privacy Practices to help you understand the limits to confidentiality and your rights regarding your medical records.

When you contact us to schedule your first session, we will provide you access to our HIPAA compliant Client Portal. Our Client Portal has all of our new client paperwork available for you to complete before you arrive for your first session. You can set up an appointment by calling our office at 217-203-2008 or emailing us.


Is Therapy Right for Me?

There are many reasons why people come to therapy. In general, if you are overwhelmed by a particular issue or are experiencing an issue that interferes with your day to day life, it’s a good idea to seek therapy. Working with a therapist can help provide insight, support, and new strategies for all types of life challenges. Therapy is right for anyone who is interested in getting the most out of their life by creating greater self-awareness and working towards change in their life.

Everyone goes through challenging situations in life. While you may have successfully navigated through other difficult times you’ve faced, there’s nothing wrong with seeking out extra support when you need it. In fact, therapy is for people who have enough self-awareness to realize they need additional support.

What Should I Expect at the First Session?

The purpose of the first session is so that you and your therapist can get to know one another and discuss your goals for your therapy. Your therapist recognizes that it may be difficult to discuss some personal information at first and you may need time to develop a relationship with your therapist before you’re ready to discuss certain topics. It’s normal to feel overwhelmed during the first session and to not know exactly where to begin. I will ask you questions to help you tell your story.

How to set up a counseling appointment:

  1. Send us an email or call us (217) 203-2008.

  2. Our office manager will send you access to our secure portal to fill out the New Client Informed Consent and Notice of Privacy Practices forms.

  3. In the first and second sessions with your therapist we’ll discuss your history, story, and goals.

  4. In the third session we’ll form a Treatment Plan to allow for a clear understanding of what to expect in counseling.

  5. You are well on your way to finding your peace within!

Email Correspondence:

If you prefer email communication, kindly inform me, and I'll promptly send you a questionnaire regarding insurance details, demographic information, and the necessary steps to begin your counseling journey at Champaign Counseling.

Complete the contact form below and our Client Care Specialist will contact you within 24 business hours. If you leave your phone number, the Client Care Specialist will attempt to call you first. If she's unable to reach you, she will send you a follow up email.
